Two different documents govern two different things, and this site keeps them apart on purpose. This one is about your medical record at the practice. The other is about what this website itself collects.
How to get the notice today.
The practice's Notice of Privacy Practices is being finalised with its own counsel, and the approved text is not published on this page yet. Until it is, you can obtain a copy the same way you always could — free, and without giving a reason:
- Ask at the front desk on the eighteenth floor and a copy is printed for you.
- Call (770) 955-0550 and ask for a copy to be posted or emailed to you.

Nothing on this page limits, replaces or restates that notice. Where this page and the practice's own notice differ, the practice's notice is the one that governs.
The notice itself.
A Notice of Privacy Practices is the practice's own legally operative statement. It sets out, in the practice's own terms, how it may use and disclose your protected health information for treatment, payment and health-care operations; the uses that require your written authorisation; your rights to inspect, copy, amend and restrict your record and to receive an accounting of disclosures; the practice's duties; who to contact with a question; and how to complain to the practice and to the U.S. Department of Health and Human Services without being penalised for it.

It is left empty deliberately. A notice of privacy practices is a dated representation to patients about how their medical information is handled, and no part of it may be drafted, adapted from a template, or approximated by anyone building this website. Ask the practice for the approved document and publish it here unchanged.
What the law requires.
This section describes the federal rule, not the practice's own terms. It is here so that you know what you are entitled to ask for while the notice above is pending.
Under the HIPAA Privacy Rule (45 CFR § 164.520), a health-care provider that transmits health information electronically must maintain a Notice of Privacy Practices, must provide it to each patient no later than the first service encounter, must make it available to anyone who asks for it, and — if it maintains a website that describes its services — must post the notice prominently on that website. It must also make a good faith effort to obtain a written acknowledgement that you received it.
Separately, 45 CFR § 164.524 and § 164.526 give you the right to inspect and obtain a copy of your own record and to ask for it to be amended, and § 164.522 lets you request restrictions on how it is used and ask to be contacted by a particular means or at a particular address. These rights exist whether or not the notice is published here yet. To exercise any of them, contact the practice using the details below.
What this website is not.
The website privacy policy is an honest, code-checked account of what the forms on thefrenchysmiles.com collect and where those details go. It is not a Notice of Privacy Practices, it does not claim to be one, and it should not be read as one. Neither document on this site asserts encryption, certification, or compliance with any regime, because none of that is verifiable from a website.
The consultation request on this site is a marketing enquiry, not a medical record. Please do not type medical history, medications, diagnoses or anything else about your health into it — the practice will take that from you directly, on a system built for it.
